Getting back on to your feet
If you have been in hospital or had an illness or injury which has meant that you are unable to carry out the usual day to day living tasks you did before there are a number of options available.
You will usually be referred by either your GP or the hospital for these services but if you would like to talk to us you can ring on 0300 300 8303 between 8:30am and 5:30pm (Monday to Thursday) and between 8:30am and 4:30pm on Friday. If your situation cannot wait you can contact the Emergency Social Worker on 0300 300 8123.
Regaining your independence (reablement)
Our reablement teams will work with you to help you to become independent again. This could be helping you to relearn some day to day tasks. This could include:
- getting up, washed and dressed
- preparing meals and snacks
- getting about in and out of the home
- doing household tasks like shopping, pension and laundry
- getting involved in social and leisure activities
Our reablement team will work with you to help you be able to do these things for yourself and build your confidence back up for a period of no longer than six weeks.
Occupational therapy
It may be that you need some equipment or some minor work to your home to help you to move around your home. Our occupational therapy team can assess what options would be suitable to help you to become independent once again.
Step Up Step Down
The Step Down service is a short term stay facility for people who have been discharged from hospital but are not yet ready to return home. They will receive 24 hour support and specialist equipment to help them get back on their feet, ready to move back home and be independent once again.
The Step Up service is for those people who can not manage at home due to an accident, fall or illness but do not need to go in to hospital. This is a short term stay facility where they will receive 24 hour support and specialist equipment to help them get back on their feet, ready to move back home and be independent once again.
