Getting back on to your feet

If you have been in hospital, or had an illness, injury or disability that has made it difficult to manage day-to-day tasks, there are services available to help you get back on your feet and live independently again.

Before you start

You can usually be referred by your GP or the hospital, but you can also contact us directly.

How we can help you get back on your feet

If you are finding it difficult to look after yourself after an illness, injury, disability or hospital stay, we can provide short-term support to help you regain independence and confidence.

Support focuses on helping you live safely in your own home and do as much as possible for yourself.

Reablement support

Reablement is short-term support to help you adapt after an illness, injury, disability or hospital stay. It helps you relearn skills and rebuild confidence so you can live independently at home.

Reablement support is usually provided for up to six weeks.

Support may include help with:

  • getting up, washed and dressed
  • preparing meals and snacks
  • moving safely around your home
  • shopping, laundry and everyday household tasks
  • returning to social and leisure activities

How reablement can help

If your health has changed quickly and you can no longer manage daily tasks, reablement can provide short-term help at home so you can continue living independently.

The team will work with you to understand what you need help with and plan the right support during the reablement period and for the future.

  • personal support such as getting in and out of bed, using the bathroom and cooking meals
  • domestic support such as shopping, cleaning and laundry

Who can get reablement

The service is for people who are unable to carry out:

  • tasks that are vital to wellbeing
  • most personal care activities
  • most domestic routines

Step Up and Step Down care

Step Up and Step Down services provide short-term care and rehabilitation with 24-hour support and specialist equipment.

Step Down

For people leaving hospital who are not yet ready to return home. You will receive short-term support to help you recover before returning home.

Step Up

For people who cannot manage safely at home due to illness, injury or a fall but do not need to be admitted to hospital.

Both services focus on helping you return home safely and independently.

Occupational therapy and home adaptations

If you are struggling to move around your home, occupational therapy can help by recommending equipment or home adaptations.

  • equipment to support daily living
  • minor works to your home
  • support towards major adaptations where appropriate
