You will need to complete this form to apply for support through the Crisis and Resilience Fund.

To complete your application, you will need to provide proof of your identity, address and financial situation. 

This may include: 

• proof of identity (such as a passport or driving licence) 

• proof of address (such as a recent utility bill or bank statement) 

• evidence of your financial situation or crisis (for example, a bill, invoice or other supporting document) 

• If you are applying for support with heating oil costs, you will need to provide: your most recent heating oil bill showing your name and address

Crisis Payments provide short-term support for residents experiencing a financial crisis or sudden financial shock. 

Each application is assessed individually based on your circumstances and the information you provide.
