Referral for children in need of help, support and protection

Do not use this form for urgent concerns.

If a child or young person is at immediate risk of harm, call 999.

Use this form when you believe a child’s needs cannot be met by a single agency alone and a co-ordinated multi-agency response is needed to support, safeguard or protect them.

Please complete all sections of the form and include as much information as possible. If you need support completing the form, contact your organisation’s safeguarding officer or the Integrated Front Door.

If you have concerns outside of these hours, contact the Emergency Duty Team on 0300 300 8123 and contact the police if needed.

All telephone referrals must be followed up in writing within 48 hours.

Before you submit your referral

Please make sure you:

  • include all relevant information, including anything already shared by telephone
  • complete all sections of the form
  • say if you do not know something
  • provide as much detail as possible to help staff make informed decisions
  • understand that forms with insufficient information may be returned to the referrer

You will not receive a copy of your referral once it has been submitted.
