Referral for children in need of help, support and protection
This form should not be used for urgent matters, which should be called through on 0300 300 8585 first. If there is an immediate risk of harm please call 999
A multi-agency referral should be made when an agency considers a child's needs cannot be met solely by that agency and co-ordinated intervention is required to promote, safeguard or protect the welfare of the child/children concerned. Please complete the form as fully as possible. If you require support then please contact the safeguarding officer in your organisation or the Integrated Front Door.
If a child or young person is at risk of serious harm contact the Integrated Front Door:

If you have concerns that a child or young person is at risk of serious harm outside of the above hours then please contact the Emergency Duty Team on 0300 300 8123 and the Police.

All telephone referrals must be followed up in writing by the referrer within 48 hours.

- Please include ALL relevant information (including info share on the telephone) and complete ALL sections
- If you don't know, please say so.
- Please complete the form as fully as possible to enable the Integrated Front Door staff to make informed decisions regarding the next course of action.
-The form could be returned to the referrer if insufficient information is provided.
