Health and social care services to be located at the Grove View Integrated Health and Care Hub

We are working with our partners Bedfordshire Hospitals NHS Foundation Trust, the Bedfordshire, Luton and Milton Keynes Integrated Care Board (ICB), East London NHS Foundation Trust and Cambridgeshire Community Services NHS Trust (CCS) and primary care networks to achieve our vision to improve the health and well-being of our residents, through greater integration and partnership working.

Below is a list of health and social care services operating from the Grove Integrated View Hub. More information on the services including their contact details can be found on the dedicated website www.groveviewhub.co.uk.

The Bedfordshire Wellbeing Service

The Bedfordshire Wellbeing Service provide consulting and counselling services for young adults and older people for residents of Bedfordshire who experience depression, anxiety, sadness, extreme shyness, obsessive behaviour, phobias, trauma, relationship difficulties including depression and common mental health symptoms or other psychological issues which are holding them back in their lives.

CAMHS

The Child and Adolescent Mental Health Service (CAMHS) provides outpatient assessments, support and treatment for children and young people up to the age of 18 experiencing moderate to severe mental health problems.

Children’s Physical health

Rapid response

The children’s rapid response service aim is to prevent children and young people who have an acute illness/deterioration in their condition from being admitted to hospital or attending accident and emergency (A&E) departments. 

Nutrition and dietetics

Our Nutrition and Dietetic Team assess and provide practical evidence-based dietary advice to help people understand the impact of diet on their health and well-being, helping people live longer healthier lives.  

Continence Specialist Services

Bedfordshire Continence Service provides advice, support, and treatment for people with continence needs. Continence advisors are trained nurses who have undertaken specialist training in continence management, and they can assess your individual continence needs and plan a treatment programme with you.

Dunstable Community Mental Health Service

The Dunstable Community Mental Health Service is a multi-disciplinary blended mental health and social care team which includes voluntary, community and social enterprise experts. The team provides holistic mental health assessment, care, and treatment within the community for those with more complex and enduring mental health needs, who can benefit from specialist support. The team promotes positive mental health and independence whilst ensuring access to appropriate support services and resources.

HUC - emergency out-of-hours 

HUC is an NHS service provider and a social enterprise. HUC provides GP Out of Hours cover as part of their Integrated Urgent Care service, which combines NHS 111 and a clinical assessment service. Between 6:30pm to 8am on weekdays, and continuously from 6:3 pm on Friday evenings to 8am on a Monday morning, including bank holidays, patients can be booked into an out-of-hours appointment at Grove View depending on the outcome of the NHS 111 assessment. To access the service, all patients should phone 111.

Heart Failure Service 

The service offers education, advice, support and NICE guidance care to patients, families and other healthcare professionals who have or treat patients with a syndrome of heart failure. The service aims to reduce admissions by treating decompensated symptoms quickly, whilst educating the patient on how to monitor for symptoms and call for help earlier.

ICASH (Sexual Health Clinic)

iCaSH is a free NHS service providing all aspects of sexual and reproductive health, including contraception, sexually transmitted infection (STI) and HIV testing and treatment.

Musculoskeletal

Circle MSK provide services for NHS patients who have Musculoskeletal (MSK) problems. This includes problems with the muscles, joints, bones, tendons, and ligaments, some nerve-related conditions and associated pain.

The Parkinson’s Disease Service Specialist nursing

The Parkinson’s Disease Service provides medication reviews and monitoring of specialist drug therapy, screening, specialist telephone advice, specialist education and support for patients and carers. Parkinson’s disease is a chronic, progressive neurological condition, that affects different aspects of life at other times of the disease process.

Path 2 Recovery (P2R)

Path 2 Recovery is a one-stop service providing advice, support, and treatment to those over 18 living in Central Bedfordshire whose lives are affected by alcohol or drug use.

Phlebotomy Community Services

Bedfordshire Phlebotomy Clinic is a bookable-only service for residents that have a Chiltern Vale GP. 

Podiatry

The Podiatry Service provides a range of specialities and techniques including assessment, diagnosis, treatment, training and advice with an emphasis on prevention and self-management in order to reduce pain and maximise improvement in foot and lower limb conditions, thus improving or maintaining the patient’s mobility and quality of life.

Priory Gardens GP Surgery

Priory Gardens GP Surgery is an NHS general practitioner’s surgery and part of the Chiltern Hills Primary Care Network. It relocated to the health and care hub and opened to patients on 2 May 2023. 

Speech and language

Adult Speech and Language Therapy (S&LT) Service provides assessment and treatment for adults (16 years and over) who have an acquired difficulty with speech, language, dysfluency (stammering), voice (including transgender voice) or swallowing.

Young Adults Independent Living Team (YAaIL)

YAaIL provides social care support for young adults with learning disabilities, physical disabilities or autism. The people we care for are aged 18-25 but also work in partnership with the Children’s Service from the age of 14.

We work in partnership with a range of local services and agencies to provide information, advice and, support to help people with disabilities develop and prepare for life as they become adults as well as enable adults with disabilities to be as independent as they can be.
