Children with Disabilities Team (CWD)

Our children with disabilities team is a specialist service that offers support to children with disabilities aged up to 18 and their families.
The service comprises a specialist team of social workers and assistants who have relevant knowledge, experience and skills. The team works closely with other services such as health, education and community organisations.
The team now has the option to complete early help assessments, as well as child and family assessments. The team manager will typically recommend which assessment is completed, based on the information on the initial referral. There are no changes to the threshold criteria for the team.
Early help assessments can typically be completed for children who are tier 3 with no safeguarding concerns. Child and family assessments will continue to be completed for children who are presenting with the highest level of need and for cases where there are safeguarding concerns.
