Care - home assessment

If you meet the eligibility criteria, care in your own home can be provided following an assessment which will be carried out by your Social Worker and Home Carer. The Home Carer will provide care as needed until the assessment is completed. This can take up to a six week period.

Staff will work closely with you to identify your personal care needs such as washing and dressing. Your domestic needs will also be taken into account and if necessary other agencies such as Occupational Therapists will be involved during this process.

Maintaining your independence will be an important part of the assessment process and staff will work with you to develop your confidence and possibly regain skills to increase your independence.

You will be consulted with throughout the period of assessment. A review meeting will be held once the assessment process has been completed.

Following the assessment we will agree the amount of care you need and will recommend agencies that will be able to provide this for you.
